Person Centered Care
This program offers full service catered to those having Alzheimer’s and other forms of dementia. This program offers your loved one the autonomy to express himself or herself without judgement. Our caregivers are equipped with appropriate skills necessary for effective communication with your loved one having dementia. For this reason, our caregivers provide their care services to your loved one with love, respect, dignity, faithfulness, and cheerful attitude. They are equipped with the skill set to enter into the worldview of your loved ones and connect with them.
We prepare a personalized plan of care for your loved one, which is constantly revised to ensure that the needs of your loved ones are met in accordance to the trajectory of their disease. To establish this plan, we use personal life story of your loved ones. This personal life story includes early childhood recollections, current preferences, stage of the disease etc., collected in collaboration with the family.
This personalized plan of care for your loved ones will include services such as ADLs (Activities of Daily Living) such as personal hygiene, bathing, grooming, incontinence care, reminiscent activities, sensory stimulating activities, real-life activities, dining service preferences, social times etc.
We also offer our person-centered care services to seniors with or without dementia. These services include, incontinence care, toileting, transferring, positioning, meal preparation, feeding, medication reminders, live-in support (24 hour-care), light housekeeping, laundry services, errands & grocery shopping, incidental transportation, recreational activities, respite care etc.

Alzheimer’s Disease & Dementia Care (ADDC) seminars to Hosana staff
At Hosana, we offer ongoing ADDC seminars. This is a training program which equips our caregivers with specialized knowledge and the skills needed to deliver high-quality care to people living with Alzheimer’s disease and other forms of dementia. We offer this 8 hours seminar through our own Memory Care Director, who is an instructor approved by the National Council of Certified Dementia Practitioners (NCCDP). We also offer ongoing refresher trainings and workshops to our caregivers on different topics about care giving such as, transfer body mechanism, client repositioning, incontinence care, toileting, hand washing, feeding etc.
We also offer some tools to family and friends of our clients on ADDC for free. We are conscious about the effects that memory impairment has on the entire family and friends. Therefore, in response to this need, Hosana offers psychoeducational sessions for the families and friends of our clients which will prepare them on the changes and challenges associated with Alzheimer’s disease and other forms of dementia. We believe that through better understanding of the disease, families and friends will develop good resilience to the trajectories of behavioral disturbance in dementia. This will help them grasp the progression of the disease and the challenges caused by the physical and mental regression that their loved ones undergo. We highly encourage Families to participate in our clients care plan. This care plan is based on the client’s preferences and stage of their memory impairment. Such a plan of care empowers our clients to reconnect with their world enabling them to find meaning in life.
Hosana is determined to support efforts to fight Alzheimer’s disease and other dementias. One way we do this is through community outreach. We offer free educational sessions to religious groups. Please call us whenever you need such a service. For those without any religious affiliation we offer free community dementia awareness in our premises.
